You must be a US resident.
You must either be uninsured or have commercial insurance. Those using Medicare, Medicaid, and any other state funded health programs are not eligible to use this program.
Certain patients in Massachusetts and California may not be eligible.
How to Apply
Visit insulinaffordability.com, scroll down, and indicate whether you use commercial insurance or pay cash (uninsured). Click Download and your savings card will be generated. Present this card when you pick up your prescription at the pharmacy. The card is limited to one refill of your prescription per month and has an annual cap of $7500. The card currently expires on 12/31/2021 and it is unclear whether Eli Lilly will continue to offer this program after that date.
LillyCares Patient Assistance Program
Eli Lilly currently offers the Lilly Cares patient assistance program which provides the following medications at no cost to qualifying customers:
There are no fees to apply or participate in this program. Processing and medication delivery times are unclear. If your application is approved, both you and your healthcare provider will receive a letter of approval. Eli Lilly will contact you to arrange a shipment of medication to your home or your healthcare provider’s office. Once approved, you will be enrolled for 12 months from the approval of your application. Medicare Part D patients will be enrolled through the end of the current calendar year. You must reapply annually.

You are a US citizen or legal resident.
You have a prescription for an Eli Lilly medication.
You must have a current healthcare provider.
You are uninsured or enrolled in Medicare Part D.
If you are eligible for Medicaid, you must have proof of denial.
You meet the household income guidelines as listed here.
How to Apply
Visit lillycare.com and choose your preferred method of application.
Complete page 2 (Patient Section) of the application. Sign the Patient Certification agreement on page 4.
Have your healthcare provider complete page 5 (Healthcare Provider/Prescriber Section) and sign page 6.
Submit the completed application and a copy of an additional document proving your income level (last year’s federal income tax record, wage statement (W2), Social Security benefits statement, etc.). If not applying online, you may submit by fax (1-844-431-6650) or mail (Lilly Cares Patient Assistance Program, P.O. Box 13185, La Jolla, CA 92039)

How to Apply
Visit insulinaffordability.com and find the “Get Savings Card” button (you may need to scroll down) and a savings card will be generated. Download or print the card and present it when you pick up your prescription at the pharmacy. Each card has a varied monthly and annual cap, so be sure to read the terms and conditions. Each card comes with a stated expiration date after which you would need to obtain a new card by reapplying.
